Here is everything you need to become a certified carrier:

• MC/MX# or DOT
  • If you’re an intrastate carrier without a DOT#, you may provide an applicable state registration number.
• Company Contact Information
• Electronic W9 (Have your TaxID available)
• Certificate of Insurance - This must be submitted from your insurance producer (agent) and meet the following requirements:
  • $100,000 cargo coverage
  • $1,000,000 auto coverage
  • $1,000,000 commercial general liability

You must meet the following requirements:

• Active Common or Contract Authority
• Safety Rating Must be Satisfactory, or None
• Authority Active for at least 180 days

• Read and Accept the Agreement(s)
